脒基硫脲和1,3-二氯丙酮环合生成2-胍基-4-氯甲基噻唑,然后和硫脲反应后,再和氯丙腈反应,生成2-胍基-4-[(氰乙硫基)甲基]噻唑,在用甲醇进行醇解,经胺化得到法莫替丁。
