以壬二酸为原料,和二咪唑亚砜反应,生成咪唑的酰化产物,该产物有很强的酰化活性,和丙二酸单甲酯反应后,再酸化脱羧生成2-[8-(甲氧基羰基)辛酰基]乙酸甲酯,再经水解脱羧生成8-氧代癸酸,和草酸二甲酯环合再脱羧生成3位取代的环戊三酮,选择性氢化其中的一个羰基为羟基,经和丙酮缩醛反应后再还原生成4-羟基-2-(庚酸甲酯-7-基)-2,3-环戊烯酮,最后和有机铝试剂反应得到米索前列醇。
